Back to School Immunization Requirements
The Tennessee Department of Health (TDOH) has issued new immunization rules and a newer Official Immunization Certificate (Form # PH-4103) that is required for school entry into certain grades. This applies to students who will be entering 7th Grade where 2 immunizations will be required.
The required immunizations will not be available at school. The immunizations and Official Immunization Certificate will only be available from a private healthcare provider or the local health department. Older versions of the immunization certificates will no longer be accepted and the newer Official Immunization Certificate showing proof of immunizations must be provided to the school prior to the start of school in August 2014. If you have any questions regarding the state requirements, please contact your private healthcare provider, the local health department, or the school nurse at the school where your child will be attending.
